Full-time or Part-time 

Seeking Certified Home Health Aides to care for clients in their homes in the Greenfield area.   HHA's are responsible for providing personal care and related household services to clients under the care of GVNA HealthCare. 



40 hours

Plans and administers physical therapy services to patients in their place of residence and in community programs in the Greenfield area according to physician orders.  Administers to patients various evaluations and therapeutic procedures while maintaining agency records.  Must be a licensed Physical Therapist.  Prefer three years of experience.  Homecare experience a plus.


GREATER GARDNER AREA (Gardner, Fitchburg, Leominster, Athol, Orange and surrounding towns):


40 hours

Responsibilities include parenting education, providing child development information, and support to first-time parents under the age of 21 from pre-natal until the child is 3 years old.  The position is based in Fitchburg, MA and requires travel within the 27 cities and towns in northern Worcester County.  Associates degree in human services, early childhood education / development or related field required.  Bachelor's degree preferred.  Must speak fluent Spanish to provide services to Spanish-only speaking families.




$1,000 Sign-on bonus

The on-call shift starts Friday at 4:30 pm until Sunday at 4:30 pm with some flexibility in hours (potential of working every other weekend). The responsibilities consist of taking calls for both Hospice and Homecare and doing PRN visits. Benefits will be offered depending on the hours agreed upon.  Hospice training provided.



Hospice/Palliative Care Program

32-40 hours

Seeking RN's with excellent clinical skills to care for Hospice and Palliative care patients.  Openings are available in the Fitchburg/Leominster, Greater Gardner and Greater Athol territories.  Training and support provided for the competent, autonomous nurses.  Individuals seeking a supportive team approach and a desire to share in end-of-life / palliative care experience should apply.  Medical/surgical experience necessary.   BSN and experience in homecare preferred.  Competitive benefit package offered.  Any full-time (40 hours) Hospice nurse is eligible for a company car.


40 hours per week

Social Worker assists the physician and other team members in understanding the significant social emotional factors related to health problems. Participates in the development of the Plan of Care, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs, and acts as a consultant to the team and other agency personnel.  Must be a LSW with a MSW or a LICSW.



40 hours

Provides support to clinical staff and assists with the accurate and timely flow of patient information and services. Must have strong computer, organizational and time management skills; analytical and critical thinking skills and ability to attend to detail.





32-40 hours

Responsible for supervision of homecare nurses, ongoing staff development, orientation, and competency evaluation.  The position includes 2 to 3 days providing direct patient care as a community health nurse with weekends in the office managing the weekend schedule and providing clinical support to the staff.  Provides assistance to the Intake, Education and Quality Improvement departments, as appropriate. The Nursing Supervisor must interact effectively with staff, patients and their families as well as coworkers from all agency departments, community resources, and physicians to facilitate quality patient care. Homecare experience required.



Could be 24 hours (weekend / one week day), 32 hours (weekend / two week days), 40 hours (weekend / three week days) or Saturday and Sunday only

Assesses and provides excellent nursing skills which contribute to treatment and rehabilitation. Assesses individual and family health needs and hazards. Training and support provided for the competent, autonomous nurse Weekend compensation rates. Two years of clinical experience required.



40 hours

Seeking a full time RN with excellent clinical skills to work in Homecare. Must have 2-5 years' experience working in a clinical setting.  Provides, arranges, teaches and supervises nursing care of clients in the home. Manages a client caseload, carries out nursing skills, contributing to treatment and rehabilitation, and assesses individual and family health needs and hazards- existing or potential. Training and support provided for the competent, autonomous nurse.  Competitive salary and benefits. Any full-time (40 hours) Homecare nurse is eligible for a company car.



32-40 hours

LPN to work full time in Homecare to provide nursing services, treatment, assessment and preventative measures by following an established plan of care. Participates in the development, implementation and evaluation of nursing care plans, and notifies RN Case Managers of any changes in patient status. Responsible for documenting visits in the computer system on a daily basis. Minimum requirement of 3 years Med/subacute/SNF experience.  Homecare experience a plus. Current LPN licensure in Massachusetts.



20 hours or per diem

Must be licensed in the Commonwealth of Massachusetts as a Speech Therapist. Responsible for the assessment and evaluation of patient care needs in treating speech and language disorders, training functionality in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Pathologist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.



24 - 32 hours per week

Homecare Social Worker participates in the plan of care, prepares clinical and progress notes and works with clients and their families.  Knowledge of community resources required.  MSW required.  LICSW preferred.  Two years working in a community setting preferred.



As needed



40 hours

Responsible for maintaining medical records and all associated information in an efficient and orderly manner.  Before discharging the records, assure records are complete.  Essential to be organized and detail-oriented.  Must have advanced computer knowledge.  Three years of experience in the medical/health field.



25 hours

The Administrative Assistant assists the Program Director with administrative tasks associated with the day-to-day operations of My Home Connection. This person will also provide support to Lifeline, ADHS and special projects as needed.



Full-time, part-time, per diem, weekends, evenings and overnights

Homecare - full or part-time - Certified Home Health Aides (HHA), Certified Nursing Assistant (CNA) or  Nursing Students enrolled in NLN-approved Nursing program with documentation of satisfactory completion of Fundamentals of Nursing course and one Medical Surgical nursing clinical rotation.  Responsible for providing personal care and related household services to clients.  

Adult Day Health Centers - per diem - Certified Home Health Aides (HHA) or Certified Nursing Assistant (CNA). Responsibilities include assisting with client hygiene, mealtime and activities. Centers are open Monday - Friday from 8:00am - 4:00pm. 

Private Duty Services - Per diem position for weekdays, weekends, evenings and overnights available - Personal Care Assistant (PCA), Certified Home Health Aide (HHA), Certified Nursing Assistant (CNA), or Nursing Students enrolled in NLN-approved Nursing program with documentation of satisfactory completion of Fundamentals of Nursing course and one Medical Surgical nursing clinical rotation.  Responsible for assisting clients in their homes with activities of daily living including personal care, meal preparation, housekeeping and socialization. Time in the home will vary by assignment and will range from 2 to 24 hours.
