Greenfield Area - Homecare
Home Health Aides

Seeking Certified Home Health Aides to care for clients in their homes in the Greenfield area. HHA's are responsible for providing personal care and related household services to clients under the care of GVNA HealthCare.
Provide evidence-based direct care on a one-to-one basis in the home setting in the Greenfield area. MA license. Two years of clinical experience required. Previous home health/VNA experience preferred. Physical Therapists that work 40 hours are eligible for a company car.

Fitchburg Adult Day Health Center
Seeking an RN to work 15 hours/wk plus per diem. Individual will be responsible for providing direct nursing care, as well as assessing and monitoring the health status of clients. Responsibilities will also include reporting any changes in status to the client's physician. Experience working with elders and disabled and 2 years supervisory experience preferred. Should be computer literate and proficient in Microsoft applications.

Private Duty Services - Per diem position for weekdays, weekends, evenings and overnights available - Personal Care Assistant (PCA), Certified Home Health Aide (HHA), Certified Nursing Assistant (CNA), or Nursing Students enrolled in NLN-approved Nursing program with documentation of satisfactory completion of Fundamentals of Nursing course and one Medical Surgical nursing clinical rotation. Time in the home will vary by assignment and will range from 2 to 24 hours.
Hospice/Palliative Care Program
Seeking RN's with excellent clinical skills to care for Hospice and Palliative care patients. Openings are available in the Fitchburg/Leominster, Greater Gardner and Greater Athol territories. Hospice Social Worker assists the physician and other team members in understanding the significant social emotional factors related to health problems. Participates in the development of the Plan of Care, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs, and acts as a consultant to the team and other agency personnel. Must be a LSW with a MSW or a LICSW.

LPN / Intake Nurse - GVNA is looking for an Intake Nurse to work 40 hours in a fast paced environment. Scheduled hours are 9:30am-6:00pm Monday-Friday with participation in a weekend rotation once every 4 weeks.
The Intake Nurse collaborates with all referral sources to provide patients with a seamless transition of care and excellent customer service. Responsibilities includes accepting and entering electronic referrals, communicating all pertinent information to appropriate personnel and requesting, reviewing and uploading discharge documents in order to ensure clinicians have the current pertinent information necessary to provide excellent patient care. The Intake Nurse acts as a primary resource to referral sources regarding services offered by GVNA and eligibility criteria.
RN Case Manager
Seeking a highly motivated RN with excellent assessment and critical thinking skills. Performs nursing assessments, plans, organizes and directs home health services contributing to the treatment and rehabilitation of patients within their home. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes. Training and support provided for the competent, autonomous nurse. Two years of clinical experience required. Previous home health/VNA experience preferred. Eligible for a leased vehicle provided by the agency.
Chronic Care Nurse
Chronic Care Nurse is responsible for the nursing assessments of non-certified clients in the home. The Nurse develops a plan of care (POC) with the physician, client and family; updating the POC and changing the POC as needed by ongoing intermittent assessment and evaluation. The Nurse provides intermittent nursing care to clients as well as teaching and supervision. Primary duties include promoting medication adherence by means of education and prefilling med planners and medication dispensing units. The Nurse provides orientation for Aides and co-visits with Aides in the home on hire and as needed to assure ongoing proficiency and skills competency. The Nurse will also be responsible for para-professional in-services under the direction of the Staff Educator. Must be an RN with a current license to practice nursing in the state of Massachusetts. Candidate should have at least one year of experience providing community-based care to the elderly population. Previous case management / care coordination experience preferred.

Must be licensed in the Commonwealth of Massachusetts as a Speech Therapist. Responsible for the assessment and evaluation of patient care needs in treating speech and language disorders, training functionality in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Pathologist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.
24 - 32 hours per week
Homecare Social Worker participates in the plan of care, prepares clinical and progress notes and works with clients and their families. Knowledge of community resources required. MSW required. LICSW preferred. Two years working in a community setting preferred.
