HOME HEALTH AIDES

Seeking Certified Home Health Aides to care for clients in their homes in the Greenfield area. HHA's are responsible for providing personal care and related household services to clients under the care of GVNA HealthCare. Home Health Aide certified preferred. Greenfield Adult Day Health Center
Seeking an RN to fill in for the Program Director while away. Individual will be responsible for providing direct nursing care, as well as assessing and monitoring the health status of clients. Responsibilities will also include reporting any changes in status to the client’s physician. Experience working with elders and disabled and 2 years supervisory experience preferred. Should be computer literate and proficient in Microsoft applications.

DIRECTOR OF HOMECARE
GVNA HealthCare is seeking an energetic, creative, forward thinking and passionate person who will be responsible for the overall direction, from both an operational and quality perspective, of our Homecare Department. Responsibilities include attracting, developing and retaining top talent, supervising and training of a high quality team, team member relations and recognition, communication, quality assurance, financial management and regulatory compliance.
Qualifications include a minimum of a Bachelor’s degree. A Master’s degree in healthcare or business preferred. Five years of management experience in homecare, hospice or related field. Must have strong organizational skills, be self-directed and have expertise in leadership, team building and interpersonal communication skills.
HUMAN RESOURCE RECRUITER / ASSISTANT
Responsible for supporting all areas of the Human Resource Department, including recruitment, orientation, personnel records, background checks and benefits administration. Associates degree or comparable experience a plus. Must possess excellent communication, organization and customer service skills. Must be able to work under pressure with a positive and professional demeanor. Minimum of 2 years’ experience working in Human Resources preferred. Networking and recruiting experience a plus. Must have excellent computer skills with a strong understanding of web-based search engines and Microsoft Office suite.
Responsible for all duties necessary for preparation, submission, collection, and maintenance of accounts receivable and the aging process of all HMO’s and third party billing. Provide back up support to the Finance Department as required for the ongoing functioning of the department. Must have excellent computer skills and a strong understanding of medical billing. Background in homecare a plus.
Candidate must have knowledge of professional occupational therapy and rehabilitation practices, including the ability to provide therapy on a one-to-one basis in a home setting. Occupational Therapy Licensure in the state of MA. Must have at least one year of experience. Competitive salary and benefits.
Hospice/Palliative Care Program
Seeking RN's with excellent clinical skills to care for Hospice and Palliative care patients. Openings are available in the Fitchburg/Leominster, Greater Gardner and Greater Athol territories. Training and support provided for the competent, autonomous nurses. Individuals seeking a supportive team approach and a desire to share in end-of-life / palliative care experience should apply. Medical/surgical experience necessary. BSN and experience in homecare preferred. Competitive benefit package offered. $1,000 Sign-on Bonus
RN- on call to work Tuesdays, Thursdays and Fridays from 4:30pm - 7:30am. Responsibilities include phone triage and home visits. This is an exempt salaried position with benefits based on a 24-hour week. Hospice experience required. Experience in Homecare a plus, but will train the right person. 2 - 3 years’ experience in an acute care setting required.
24 – 32 hours per week
Social Worker participates in the plan of care, prepares clinical and progress notes and works with clients and their families. Knowledge of community resources required. MSW required. LICSW preferred. Two years working in a community setting preferred.
RN CASE MANAGER
Seeking a full time RN with excellent clinical skills to work in Homecare. Must have 2-5 years' experience working in a clinical setting. Gives, arranges, teaches and supervises nursing care of clients in the home. Manages a client caseload, carries out nursing skills, contributing to treatment and rehabilitation, and assesses individual and family health needs and hazards- existing or potential. Training and support provided for the competent, autonomous nurse. Competitive salary and benefits.
To work in the Homecare Department. Must have excellent computer skills, customer service skills and ability and willingness to work in a fast paced environment. Must be able to handle multiple tasks at the same time. Ability to work under pressure and handle stress.
HEALTHY FAMILIES HOME VISITORS
The Healthy Families program is looking for 3 enthusiastic and caring Home Visitors. Responsibilities will include the outreach to first-time parents providing home-based support, parenting education, screenings, advocacy and referrals to appropriate community services. Experience with young families, infants, toddlers and knowledge of community resources a plus. Reliable transportation required. Bachelor’s degree in Human Service, Childhood Education or related field required.
Seeking Physical Therapists to work in the greater Gardner area. Candidate must have the ability to treat a variety of diagnoses and provide therapy on a one-to-one basis in a home setting. Physical Therapy Licensure in the state of Massachusetts required. Competitive salary and benefits.

Homecare - Certified Home Health Aides and/or CNA’s. Responsible for providing personal care and related household services to clients. Full-time or part-time.
Adult Day Health Centers - Aide responsibilities include assisting with client hygiene, mealtime and activities. Centers are open Monday – Friday from 8:00am – 4:00pm. Per diem positions available.
Private Duty Services - Personal Care Assistant (PCA), Certified Nursing Assistant (CNA) or Home Health Aide (HHA). Responsible for assisting clients in their homes with activities of daily living including personal care, meal preparation, housekeeping and socialization. Time in the home will vary by assignment and will range from 2 to 24 hours. Per diem position for weekdays, weekends, evenings and overnights available.
