Greenfield Area

 

Home Health Aides
Full-time or Part-time

Seeking Certified Home Health Aides to care for clients in their homes in the Greenfield area.   HHA's are responsible for providing personal care and related household services to clients under the care of GVNA HealthCare.

 

Licensed Practical Nurse (LPN)
LPN to work in Homecare to provide nursing services, treatment, assessment and preventative measures by following an established plan of care. Participates in the development, implementation and evaluation of nursing care plans, and notifies RN Case Managers of any changes in patient status. Responsible for documenting visits in the computer system on a daily basis. An LPN would work weekdays with a weekend rotation. Training and support provided.

 

Physical Therapist
Full Time - Plans and administers physical therapy services to patients in their place of residence and in community programs in the Greenfield area according to physician orders.  Administers to patients various evaluations and therapeutic procedures while maintaining agency records.  Must be licensed Physical Therapist.  Prefer three years of experience.  Homecare experience a plus.

 

Program Aide - Adult Day Health  Center
20 hours - Certified Home Health Aides (HHA) or Certified Nursing Assistant (CNA). Responsibilities include assisting with client hygiene, mealtime and activities.

 

Program Director/Service Coordinator - Adult Day Health Center
20 hours - The Program Director/Service Coordinator directs and supervises the day-to-day operations of this program. The individual who holds this position develops and implements a plan of care which includes coordinator of services to meet the psychosocial needs of the client.

 

Registered Nurses (RN)
We are looking for Registered Nurses to give nursing care in the client’s home. This position manages a client caseload, carries out nursing skills, contributing to treatment and rehabilitation, and assesses individual and family health needs and hazards-existing or potential. Involve the patient and/or caregiver in the plan of care focused on achieving maximum independence. Provide cost effective care assuring that services and referrals are goal directed.

40 hours a week position with weekend rotation.  At this time weekend rotation is every fourth weekend (subject to change based on staffing). Also have openings for 24, 32 or 40 hours to work every weekend with hours during the week.  This position is paid at a premium rate. A leased vehicle is available to Registered Nurses working 40 hours a week. Training and support provided.

 

Greater Gardner Area (Gardner, Fitchburg, Leominster, Athol, Orange and surrounding towns):

 

AIDES
Full-time, part-time, per diem, weekends, evenings and overnights

  • Homecare - full or part-time - Certified Home Health Aides (HHA), Certified Nursing Assistant (CNA) or Nursing Students enrolled in NLN-approved Nursing program with documentation of satisfactory completion of Fundamentals of Nursing course and one Medical Surgical nursing clinical rotation. Responsible for providing personal care and related household services to clients.

 

  • Adult Day Health Centers - Per diem - Certified Home Health Aides (HHA) or Certified Nursing Assistant (CNA). Responsibilities include assisting with client hygiene, mealtime and activities. Centers are open Monday – Friday from 8:00am – 4:00pm.

 

  • Private Duty Services - Per diem position for weekdays, weekends, evenings and overnights available - Personal Care Assistant (PCA), Certified Home Health Aide (HHA), Certified Nursing Assistant (CNA), or Nursing Students enrolled in NLN-approved Nursing program with documentation of satisfactory completion of Fundamentals of Nursing course and one Medical Surgical nursing clinical rotation.  Responsible for assisting clients in their homes with activities of daily living including personal care, meal preparation, housekeeping and socialization. Time in the home will vary by assignment and will range from 2 to 24 hours.

 

Clerical Support For The Hospice Program
Full time - Provides support to clinical staff and assist with the accurate and time flow of patient information and series. Must have strong computer, organization and time management skills; analytical and critical thinking skills and very detail oriented.

 

Home Visitor / Bi-Lingual (Spanish)
Full Time - The Healthy Families Bilingual Home Visitor is responsible for parenting education, providing child development information, and support to first-time parents under the age of 21 during pregnancy to age 3.  The position is based in Fitchburg, MA and requires travel within the 27 cities and towns in northern Worcester County. Associates degree in human services, early childhood or related field required. Bachelor’s degree preferred. Must have dependable transportation.

 

Intake Nurse - LPN
Full time - Scheduled hours 9:30am-6:00pm Monday-Friday with participation in a weekend rotation once every 4 weeks. The Intake Nurse collaborates with all referral sources to provide patients with a seamless transition of care and excellent customer service. Minimum requirement must be LPN with excellent communication and customer service skills. Must have the ability to organize and prioritize tasks and able to work in a stressful environment. Excellent computer skills required.

 

Occupational Therapist
Full time - Plans and administers occupational therapy services to patients in their place of residence and in community programs according to physician’s orders.  Administers to patients various evaluations and therapeutic procedures while maintaining agency records.  Patient population may range from birth to over 100 years of age with impairments not limited to: muscular skeletal, neurological, orthopedic resulting in functional limitations and or disabilities due to injury, illness, disease or other causes.

 

Physical Therapist
Full time - Plans and administers physical therapy services to patients in their place of residence and in community programs in the Gardner area according to physician orders.  Administers to patients various evaluations and therapeutic procedures while maintaining agency records.  Must be licensed Physical Therapist.  Prefer three years of experience.  Homecare experience a plus.

 

Registered Nurses (RN) - Homecare
We are looking for Registered Nurses to give nursing care in the client’s home. RN openings: 40 hours a week position with weekend rotation.  At this time weekend rotation is every fourth weekend (subject to change based on staffing). A leased vehicle is available to Registered Nurses working 40 hours a week. Training and support provided.

RN openings:  24, 32 or 40 hours to work every weekend with hours during the week.  This position is paid at a premium rate. Training and support provided.

 

Speech Therapist
20 hours or per diem - Must be licensed in the Commonwealth of Massachusetts as a Speech Therapist. Responsible for the assessment and evaluation of patient care needs in treating speech and language disorders, training functionality in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Speech Pathologist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

 

Social Worker - Homecare and/or Hospice
Social Worker assists the physician and other team members in understanding the significant social emotional factors related to health problems. Participates in the development of the Plan of Care, prepares clinical and progress notes, works with the family, utilizes appropriate community resources, participates in discharge planning and in-service programs, and acts as a consultant to the team and other agency personnel.  Must be a LSW with a MSW.  LICSW, Licensed independent clinical social worker, preferred.  Two years working in a community setting preferred.
